They've identified this as a problem since a large amount of content has slipped through the system. Likely the missed content is due to people slighting pitching audio up, adding a border around the image, or other methods to avoid detection. This is a popular solution people on YouTube use to bypass the content ID system, as you can see in this video screenshot.
